MyEMH Record FAQs


How can I sign up?

If you were given your Medical Record Number and you provided us with your email address at your last hospital visit, you can enroll by logging into myrecord.emhc.org.

If you requested enrollment through Elmhurst Memorial Hospital at the time of your visit, please check your email and complete your enrollment using the link that was sent to you.


Some of my information is wrong. How do I correct this?

If you notice an error in your personal information, such as address and phone number, please send us a change request through the portal. The change request can be found by clicking Profile, and then Update Profile. Any changes requested will be reviewed and, if accepted, will be updated in the portal profile information. This will not however update your Electronic Medical Record that is maintained by Elmhurst Memorial Hospital. Upon your next visit for services at Elmhurst Memorial Hospital please bring photo ID with correct information and we will update the Elmhurst system at that time.

If you see information in your medical history that is incorrect, please notify the Health Information Management department at 331-221-6755. Request an Amendment of Health Information form to be mailed to you home address or via the MyEMH Record portal page.


What if I need help?

Please notify the Health Information Management department (Medical Records) at 331-221-0503 or at contactus@emhc.org. The Health Information Management department is available Monday through Friday, 9 am - 4:00 pm.


How do I communicate my medical concerns?

MyEMH Record is not used to communicate medical concerns to your doctor. IF YOU ARE EXPERIENCING AN URGENT MEDICAL PROBLEM, PLEASE CALL 911 OR YOUR PHYSICIAN'S OFFICE IMMEDIATELY.


How do I see my test results?

Your test results are available by clicking on the Health Record button on the portal home page (click here to log in). Once you click on the Health Record button you will be taken to the Health Record page where you will see buttons on the right labeled Results for access to your Laboratory Results.


How do I get the complete set of my medical records?

You can obtain your medical records by submitting an Authorization to Use and Disclose Protected Health Information (PHI). This form can be obtained via the MyEMH Record Patient Portal, on the emhc.org web site Privacy and Consent form page, or by contacting Health Information Management (Medical Records) at 331-221-6755. Print copy fees do apply


I forgot my password.

Enter in your login ID on the MyEMH Record Portal sign-in page and click forgot password. A new password will be sent to your email account on file.


How do I reset my password or change it if I feel it has been compromised?

Within the MyEMH Record Profile you have the ability to change password.


How can I update my email address in MyEMH Record?

Updates are taken at the time of your next registration for services provided at Elmhurst Memorial Hospital.


What information can I see in MyEMH Record?

You can see information from your medical record such as laboratory test results, medications, allergies, immunizations, medical history, current health issues and due dates for preventive care. You may also print a Health Summary for your records.


Is there a fee to use MyEMH Record?

No! MyEMH Record is a free service offered to our patients.


How is MyEMH Record secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through personal IDs and passwords. MyEMH Record uses the latest encryption technology. All MyEMH Record messaging is done while you are securely logged on to our website.


Can I also see my family members' health records?

Yes, this is called Proxy Access. For a child under 12, this allows a parent or legal guardian to log into their personal MyEMH Record account, and then view information regarding their child. After the child reaches age 12, Elmhurst Memorial Hospital requires parents/guardians to obtain an authorization signed by the child for access to the child's medical record information.

Once a child turns 18, a parent can only access the child's patient medical record information with the patient's authorization. It is also possible to request access to another adult's health record if you help manage that adult's medical care. What if I need more information than is available through MyEMH Record?

If you need additional information than what is available through MyEMH Record, please contact the Health Information Management department at 331-221-6755.


How can I reschedule my appointment through MyEMH Record?

To reschedule your appointment, please contact scheduling at 331-221-0005. Central scheduling hours of operation: Monday - Friday 7:30 a.m. - 6:00 p.m.; Saturday 8:00 a.m. - 1:00 p.m.


How can I request appointments that are not listed in MyEMH Record?

You are able to request other appointments that are not listed by contacting scheduling at 331-221-0005. Central scheduling hours of operation: Monday - Friday 7:30 a.m. - 6:00 p.m.; Saturday 8:00 a.m. - 1:00 p.m.


How far back does MyEMH Record populate?

You can see your information from MyEMH Record regarding any tests you undergo April 04, 2014 forward. We take great care to ensure your health information is kept private and secure. How can I pay my bills via MyEMH Record?

You may pay your bill online at www.healthbillpay.com/elmhurstmemorial. If you need to discuss setting up a payment plan, please contact Elmhurst Memorial Health Care toll-free at 866-756-8353.


Why do we get New Activity emails?

You will receive new activity emails whenever you have a new update to your MyEMH Records.

What is available for Elmhurst Memorial Affiliated Physician Practices patients?

While the MyEMH Record patient portal is available to Elmhurst Memorial hospital patients, the following Elmhurst Memorial physician practices have changed to a new clinical system and offer the MyChart patient portal to their patients. Please visit the web sites below to learn about the MyChart patient portal and how to sign up.
