If you were given your Medical Record Number and you provided us with your email address at your last hospital visit, you can enroll by logging into myrecord.emhc.org.
If you requested enrollment through Elmhurst Memorial Hospital at the time of your visit, please check your email and complete your enrollment using the link that was sent to you.
If you notice an error in your personal information, such as address and phone number, please send us a change request through the portal. The change request can be found by clicking Profile, and then Update Profile. Any changes requested will be reviewed and, if accepted, will be updated in the portal profile information. This will not however update your Electronic Medical Record that is maintained by Elmhurst Memorial Hospital. Upon your next visit for services at Elmhurst Memorial Hospital please bring photo ID with correct information and we will update the Elmhurst system at that time.
If you see information in your medical history that is incorrect, please notify the Health Information Management department at 331-221-6755. Request an Amendment of Health Information form to be mailed to you home address or via the MyEMH Record portal page.

MyEMH Record is not used to communicate medical concerns to your doctor. IF YOU ARE EXPERIENCING AN URGENT MEDICAL PROBLEM, PLEASE CALL 911 OR YOUR PHYSICIAN'S OFFICE IMMEDIATELY.
Your test results are available by clicking on the Health Record button on the portal home page (click here to log in). Once you click on the Health Record button you will be taken to the Health Record page where you will see buttons on the right labeled Results for access to your Laboratory Results.
You can obtain your medical records by submitting an Authorization to Use and Disclose Protected Health Information (PHI). This form can be obtained via the MyEMH Record Patient Portal, on the emhc.org web site Privacy and Consent form page, or by contacting Health Information Management (Medical Records) at 331-221-6755. Enter in your login ID on the MyEMH Record Portal sign-in page and click forgot password. A new password will be sent to your email account on file.
Within the MyEMH Record Profile you have the ability to change password.
Updates are taken at the time of your next registration for services provided at Elmhurst Memorial Hospital.
You can see information from your medical record such as laboratory test results, medications, allergies, immunizations, medical history, current health issues and due dates for preventive care. You may also print a Health Summary for your records.
No! MyEMH Record is a free service offered to our patients.
We take great care to ensure your health information is kept private and secure. Access to information is controlled through personal IDs and passwords. MyEMH Record uses the latest encryption technology. All MyEMH Record messaging is done while you are securely logged on to our website.
Yes, this is called Proxy Access. For a child under 12, this allows a parent or legal guardian to log into their personal MyEMH Record account, and then view information regarding their child. After the child reaches age 12, Elmhurst Memorial Hospital requires parents/guardians to obtain an authorization signed by the child for access to the child's medical record information.
Once a child turns 18, a parent can only access the child's patient medical record information with the patient's authorization. It is also possible to request access to another adult's health record if you help manage that adult's medical care.
