Residential Home Health and Hospice:Residential Home Health Services
Elmhurst Memorial Home Health is now a part of Residential Home Health. We offer patient-specific in-home skilled nursing and therapy services that are coordinated with your primary care physician.
Residential’s clinical care team includes:
- Physical Therapists
- Occupational Therapists
- Speech Therapists (Speech Language Pathologists)
- Medical Social Workers
Examples of skilled nursing care include changing dressings, teaching about disease processes and conditions, reconciling medications, giving IV drugs, shots or tube feedings, and more. Our clinicians are dedicated to coordinating and delivering expert services to ensure patients receive appropriate and exceptional care, improving their quality of life, safety and independence at home.
Many people are living with multiple conditions and the care required to manage their health can be overwhelming. Residential offers specialized programs that address issues associated with several common chronic diseases. Our programs are not a “one-size-fits-all” solution; we build a plan of care specific to each of our patient’s health goals and requirements.
Call (866) 902-4000 and discuss with a nurse how these programs may be coordinated to address your specific health situation.

If you’re a caregiver, it can be intimidating and confusing to try to balance caring for your loved one and maintaining day-to-day responsibilities like a job or family of your own. As you attempt to match available resources with the needs of your loved one, the thought of another hospitalization can be especially daunting.
Did You Know?

- 33% of all Medicare patients are readmitted to the hospital within 90 days
