CHAMP - Cardiopulmonary Disorders
CHAMP stands for Cardiopulmonary Hospital Admit Management Program and incorporates Cardiocom telehealth, cardiac rehab, and behavior modification to prevent hospital readmissions.
Joint Replacement Home Therapy - Orthopedic Care
The orthopedic therapy program helps patients recover faster after surgery so they can get back to spending time doing the things they love.
StepWise - Fall Awareness and Prevention
The StepWise fall awareness and prevention program increases independence and safety by improving mobility, balance, and strength.
MindCare - Dementia and Alzheimer’s Care
MindCare is a dementia care program that increases patient participation in everyday activities and reduces caregiver stress.
LSVT® BIG and LSVT® LOUD - Neurological Care
Certified in LSVT® BIG and LSVT® LOUD, Residential Home Health’s therapists leverage these innovative treatments that are proven to help patients with Parkinson’s and other neurological disorders, such as stroke and multiple sclerosis, walk and talk to their fullest potential.
Speech Therapy - Addressing Speech, Language, Swallowing, and Cognitive/Memory Conditions
Residential’s Speech Therapy program assists patients with motor speech disorders, swallowing difficulty, loss of language ability, and cognitive and memory conditions. Services include intervention of disorders such as Parkinson’s disease, stroke, ALS, head injury, dementia, COPD or CHF.
Comfort Path - Palliative Care
Comfort Path is a palliative-focused home care program designed for patients who are still seeking curative treatment options or who are not quite ready to transition to hospice. Residential Comfort Path provides physical, emotional and psychosocial support to improve quality of life for patients facing a debilitating illness.
