Enrolling in Telecare

Telecare is peace of mind for seniors or disabled persons who live alone and their families. Elmhurst Memorial Hospital's Volunteer Services Department offers this free, daily phone service to check on the safety and well-being of those who live alone.

Here's how it works: you call us each day between 8 a.m. and Noon. Our service is available either Monday through Friday or Monday through Saturday. Please note that our service is not available on Sundays or national holidays.

If you do not call and we cannot reach you, we will call friends, neighbors or relatives you previously designated in case of emergency. If your contacts don’t know where you are and are unable to reach you, Telecare will call your local police for assistance.

Please fill out the contact form below. Once we receive your information, we will contact you to start the Telecare enrollment process. (Please be aware that filling out this form does not immediately enroll you in the Telecard program.)
