By providing an atmosphere of wellness and recovery, with skilled rehabilitative or nursing care, we provide an ideal alternative for your transitional needs between being in the Hospital and returning home. Through our partnership with Marianjoy Rehabilitation Hosptial, we are able to provide these services to our community, close to home. Our Transitional Care Center is fully certified for Medicare/Medicaid reimbursement and accredited by the Joint Commission on Accreditation of Healthcare Organizations.
